Medication Refill Policy
All prescriptions and authorizations for refills must be requested during normal office hours at our medical office. We are unable to process refill requests and authorizations through our Immediate Care. We will not dispense medications over the telephone unless you have been seen by one of our providers within the last six months.
With the many prescription drug plans available today, we cannot know the details of each plan. It is your responsibility to understand how your prescription drug plan works and what method should be used for processing your prescriptions. Please let us know about medications needing refills with enough advance notice to avoid any lapse in taking your medication.
Patients are advised to request medication refills by contacting their pharmacy three to four days before running out of medication.
Medical Records Release Policy
We are unable to send any medical records out of our office without a signed authorization from the patient or their guardian. Once a completed form is received, the request will be promptly processed. The only exception to this are when we refer you to a specialist for care or when a school or daycare sends us a faxed request for immunization records.

Payment Policy
At the time of scheduling your appointment, you will be asked to provide your health insurance information. This allows us to keep our records up to date. We also make every attempt to verify your eligibility prior to your visit. Insurance carriers require patients to present their insurance card at each visit and to pay their co-pay at the time of service. With this in mind, we kindly ask that you keep our office informed of all health insurance coverage you have. Ultimately, the patient or parent/guardian is responsible for any and all charges incurred. Providing our office with the correct insurance carrier(s) helps us keep your out-of-pocket costs as low as possible.
Patients without health insurance may schedule an appointment to see our Care Coordinator prior to their visit. The Care Coordinator will assist in researching options for Medicaid eligibility, insurance through the Marketplace, or utilizing our Sliding Fee Discount Program. The Sliding Fee Discount Program is based on the Federal Poverty Guidelines.
